Lay Summary: This study aims to explore the perspectives of Transgender and nonbinary youth and their caregiver(s), both pre- and post-transition, to identify needs and concerns regarding transition of gender-affirming care from pediatric to adult settings including perspectives of caregivers gives a more complete picture of youth preparedness. Findings from this project will inform the development of transition tools and workflows to facilitate and improve the transition from pediatric to adult care for this population.
