The study aims to develop a transition program that will improve the process of transitioning to adult care for IBD patients across Canada and internationally. Other studies have shown that patients sometimes have worsened health outcomes (they are more likely to become ill) after a transfer of care. We hope that participating in this transition program will help you to feel more comfortable and prepared for when the time comes for you to transfer to an adult specialist.
