In neonates, iatrogenic hypothyroidism has been described following topical, parenteral, enteral, or lymphatic iodine exposure.1–3 Excess iodine paradoxically blocks thyroid hormone synthesis in a phenomenon known as the Wolff-Chaikoff effect. In the healthy, mature postnatal thyroid gland, there is escape from this inhibition; however, in the immature thyroid gland, this escape mechanism is not well developed, putting neonates and especially premature neonates at risk of hypothyroidism.
